111 online is a fast and convenient alternative to the 111 phone service and provides an option for people who want to access 111 digitally. 

Your needs will be assessed and you will be given advice about whether you need:

  • Treat yourself at home
  • Go to a Primary Care Centre

If you need face to face medical attention you may be asked to attend a Primary Care Centre.

How to book your home eye test & check eligibility

1. Check if you’re eligible here

You’ll be eligible for a free home eye test if you qualify for a free NHS-funded eye test and are unable to leave home unaccompanied due to a physical or mental illness or disability.

2. Get in touch

You can choose how you would like to request your home visit. Call us directly on 0808 134 9669 to arrange your appointment or fill out our form below. Once you have submitted your request online, someone will be in touch.

We offer both FACE to FACE and REMOTE SUPPORT (where assistance will be given over the phone or video calls). *Young people under the age of 16 will require parent / guardian consent to download the Zoom application onto personal devices
